Full-Time | Reports to Director of Programs
Pregnant with Possibilities Resource Center (PPRC) is seeking a Community Care Coordinator to provide holistic, culturally responsive support to pregnant and postpartum participants. This role serves as a trusted point of contact, connecting families to healthcare, social services, and community resources while advocating for equitable maternal and infant health outcomes.
Key Responsibilities:
Conduct intake assessments and develop individualized care plans
Connect participants to healthcare, housing, employment, and social services
Facilitate workshops, support groups, and community outreach
Advocate for participants and ensure trauma-informed, culturally competent services
Qualifications: Associate’s or bachelor’s degree (or equivalent experience), 2+ years in case management or community health, strong knowledge of maternal and child health, and excellent communication skills. Bilingual skills and CHW certification are a plus.

What You’ll Do
Facilitate youth and parent-focused curricula through workshops, sessions, and activities.
Recruit, engage, and support participants while fostering strong relationships.
Coordinate outreach with schools, counselors, and community partners.
Collect and track participant data to measure program success.
Organize special events, guest speakers, and activities to enhance learning.
Represent PPRC in the community and contribute to a culture of growth and empowerment.
What We’re Looking For
Bachelor’s degree in social work, psychology, education, or a related field (or equivalent experience).
1–2 years’ experience in program facilitation, youth development, or teaching.
Strong communication and organizational skills with the ability to inspire and engage.
Comfortable managing group dynamics and fostering inclusive spaces.
Valid driver’s license, reliable transportation, and flexibility to work some evenings/weekends.
Bilingual or multilingual skills (especially Spanish) are a plus!
